To assist the processing section in directing the requests to the
appropriate unit/locations, we recommend that a separate form be
completed for each type of file being requested: Eg.(1) one for RCMP
Service and Personnel files, (2) one for the RCMP Medical file, and (3) one for any other type of personal information you might be seeking.
Please ensure that rank, Regimental and/or HRMIS number are included
in the details of the information being sought. If the member is retired, this should be indicated as well.
